This Transformational Tours & Retreats Booking Agreement is made on Select Date (date) between Transformational Tours & Retreats (“TT&R”) and First and Last Name (Guest legal name as appears on passport) (“Guest” or “You”).

TRANSFORMATIONAL TOURS’ BOOKING TERMS & CONDITIONS

Please note: Each Guest on this tour must SIGN this contract. Once signed, You will receive a copy of the contract by email.

TOUR PRICE INCLUDES

ACCOMMODATIONS 10 days/9 nights are provided as listed below. Tour Price is per person and includes accommodations at deluxe hotels based on double occupancy.

• 05 nights at Anantara Riverside Bangkok Resort or similar
• 04 nights at Na Nirand Romantic Boutique Resort or similar

OTHER INCLUSIONS

• All touring and transfers in private air-conditioned motor coach
• Entrance fees to all visits included in tour
• 2 Domestic flights within Thailand
• Daily breakfast
• 5 Lunches
• 4 Dinners
• All hotel taxes, service charges and government sales tax
• Luggage handling at accommodations.
• Tips for guides, drivers, waiters for group meals, and hotel staff

TOUR PRICE EXCLUDES

• International air tickets
• Drinks & meals not listed in this Booking Agreement.
• Laundry, telephone calls, or other items of a personal nature
• Spa treatments (optional)
• Travel and health Insurance
• Any rate increases in entrance fees, tickets, or domestic airline charges may result in additional charges to Guest. TT&R has no control over government controlled fees. Any rate changes will be communicated to Guest if, and when, TT&R is apprised.

TOUR PRICE

• $6,497.00 per person covers the complete land package as indicated in the itinerary, based on double occupancy
• Single Supplement - $1,599.00

DEPOSIT

A $997.00 non-refundable deposit is due upon sign-up and holds your space.

PAYMENT OPTIONS

• PAY IN FULL $6,497.00
• MONTHLY PAYMENTS: $997.00 Deposit monthly auto credit card payments. Amount of monthly payment depends on sign up date, with payments ending on August 15, 2023. (Please contact Holly Johansen, Client Coordinator at holly@toursandrtreats.com or (888) 600-7541 to arrange monthly credit card payments.)
• 3 INTERIM PAYMENTS: 3 payments of $1,834.00 due no later than May 15, July 1, August 15, 2023 (a reminder email will be sent out prior to payment due date).

HOW TO PAY

• Credit Cards: Visa, MasterCard, Discover and American Express are accepted.

Domestic flights may increase if You book after June 30, 2023. Departure taxes, security and fuel surcharges are subject to change without notice.

OPTIONAL EXTENSION

November 13-17 Phuket Beach 4-night Extension

INCLUSIONS

All Inclusions and Exclusions to Main Tour apply, and:

• 04 nights’ Accommodations at Amari Phuket in Oceanfront Room
• 1 Domestic flight within Thailand
• Daily breakfast (4)
• Speedboat Tour or similar
• 1 lunch

PAYMENT

$1,534.00 total Extension price, based on double occupancy. Single supplement is $475.00.

If You elect to join this extension, Tour Price will be amended as follows:

You can elect to pay in full, make additional monthly payments to auto credit card or make 3 payments of $511.00, per dates listed above, (with single supplement 3 payments of $159.00, or on monthly plan).

This Extension is based on a minimum of 20 guests, and price may increase slightly if fewer guests attend.

CANCELLATION:

DEPOSIT IS NON-REFUNDABLE IN THE CASE OF CANCELLATION.

Refunds due to cancellation:

• Guests cancelling prior to May 1, 2023, receive a full refund of Tour Cost paid, less the initial $997.00 deposit.
• Guests cancelling prior to June 1, 2023, receive a 50% refund of Tour Cost paid, less the initial $997.00 deposit.
• Guests cancelling on or after August 1, 2023, no refund shall be issued.

TT&R strongly encourages Guest to purchase Trip Insurance. Please contact TT&R for assistance in this process.

COVID or other issues outside of TT&R Control: If Tour postponement becomes necessary due to Pandemic travel restrictions or other reasons outside the control of TT&R, Guest shall receive a full credit for rescheduled tour date.

DOUBLE OCCUPANCY/ROOMMATES/SINGLE SUPPLEMENTS

Tour Price is per person, based upon double occupancy.

You are encouraged to sign-up with a roommate. TT&R will provide roommates, upon request, and when possible. If no roommate is available when the tour closes 75 days before travel, Guest may be required to pay a single supplement, so Guests are strongly encouraged to sign up early or with their own roommates.

INTERNATIONAL FLIGHTS

TT&R is happy to review your itinerary prior to purchase on a complimentary basis. Additionally, if requested TT&R will assist with international flight arrangements. TT&R can be reached via website, toursandretreats.com or at (888) 600-7541. TT&R Booking Fee is $50.00 per person for a coach flight and $100.00 Booking Fee per person for either business or first-class flight. This Booking Fee is in addition to the ticket cost, is paid at time of ticket purchase, and is non-refundable. Guest is responsible for flight management, seat assignments, check-in, transit to departing airport, documentation, and potential COVID-19 test requirements. TT&R accepts no responsibility for international flights beyond booking your air travel.

TRAVEL DOCUMENTS

PASSPORT: Guest is required to have a passport valid for at least six months AFTER the date of arrival. If arriving at start of tour, November 4, 2023 entering Thailand (your passport MUST be valid through at least May 4, 2024) or You must renew before Tour. NOTE: This rule is enforced by the Thailand Government.

VISA: U.S. citizen tourists entering Thailand for fewer than 30 days do not require a visa.

BAGGAGE

Please check with your international airline for specifics on the size and weight of your allowed baggage. The in-country flights will limit your luggage to one piece (maximum of 44 pounds), and one carry-on that fits under the seat, (maximum 11 pounds).

TRAVEL INSURANCE

TT&R highly recommends travel insurance for the Tour and can provide access to a variety of travel insurance options. If Guest seeks assistance purchasing travel insurance, an appointment with advisor is available via website, toursandretreats.com, or by calling 888-600-7541. If Guest does NOT obtain travel insurance on their own or with TT&R, Guest must sign a waiver indicating this.

Important: If Guest wants “cancel for any reason” travel insurance, Guest MUST purchase it within 15 days of TT&R receipt of Deposit.

Guest understands that TT&R is not an insurer, does not provide travel insurance or provide any insurance coverage for Guest or Tour. TT&R may offer suggestions and or access to travel insurance, as a courtesy. TT&R is not endorsing any travel insurance policy or advising Guest on the veracity of the travel insurance purchased, and TT&R shall not be responsible for Guest’s choice of policy or coverage purchased and if purchased through TT&R. Guest shall not rely on any representations and/or opinions offered by TT&R. It is Guest’s responsibility to review and thoroughly understand the insurance coverage they purchase. Guest understands that medical expenses/and or cancellation relating to COVID or other Pandemic Outbreaks may not be covered, per the insurance policy, and that these changes may alter without notice and through no control of TT&R.

MEDICAL CARE

Guest acknowledges some locations, destinations or regions of Tour may be remote or have nonexistent or inadequate medical care available. Traveler attests they have either travel/medical insurance or adequate financial resources for any medical care required on Tour.

EPIDEMICS/PANDEMICS

Guest acknowledges in the case of an Epidemic or Pandemic, which Guest agrees is a widespread occurrence of an infectious disease (collectively “Pandemic”), that governmental and territorial responses, domestically and internationally, vary and are subject to change without notice, including immigration restrictions which may be instituted before or during Tour that may impede Guest’s ability to enter or exit Tour destination(s) as planned. Guest understands that it is their responsibility to check the latest domestic and international travel information, warnings restrictions, and rules regarding any Pandemic and understands the risks, and that Guest is expected to follow CDC guidelines to protect fellow travelers.

HEALTH AND MOBILITY DURING TOUR

In purchasing your tour, you attest that you are physically fit for the requirements of the duration and locations indicated in the itinerary. You must ensure You are medically sound and physically able to travel. Tour may include rough terrain, extensive walking over cobblestone streets, uneven pavement, steps and/or locations which may not be easily accessible by wheelchair, among others. You must make your own decisions about these experiences and participate only in activities that suit your physical ability. Please speak with your medical provider if you have questions about your abilities.

INDEMNITY

Guest agrees to hold TT&R harmless for any financial loss, penalties or fees associated with, and not limited to: travel restrictions, entry denial or delays, illness, death, quarantine rules or directives, proof of recent testing results or on-the-spot testing related to a Pandemic, mandatory mask wearing, social distancing measures, event/itinerary cancellation or modification, interruption of services of suppliers, hotels, airlines, cruise lines, excursion and tour providers, and denial of insurance claim as it relates to Pandemic or any other claim under the policy. Guest further agrees not to institute a credit card dispute or “chargeback” against TT&R for any penalties or fees.

RESPONSIBILITY

TT&R has planned the Tour set forth in this Booking Agreement. Hotels, airlines, and other suppliers providing tour services are independent contractors and are not the agents, employees, or servants of, or joint venturers with TT&R. All certificates and other travel documents for tour services issued by us or our booking agent, EXO Travel, or by Gifted Travel Network, are subject to the terms and conditions specified by the supplier and the laws of the countries is which the services are supplied. By utilizing the services of TT&R, the Guest agrees that TT&R, EXO Travel, Oneness for All, and Gifted Travel Network, shall not be liable for any delay, inconvenience, loss of employment, upset, disappointment, distress or frustration, whether physical or mental, resulting from the act or omission, and shall not be liable for any damage, loss, claim, cost or expense arising out of personal injury, accident, or death, loss, damage or delay of baggage or other property, resulting from: (a) the act or omission on the part of company or any person including but not limited to TT&R, Halle Eavelyn, Sandy Babin, Oneness for All, Howard Caesar, or EXO Travel, or (b) acts of God, sickness, theft, labor disputes, mechanical breakdown, government actions, weather, or any cause beyond TT&R or EXO Travel’s direct control.

No person, other than a representative of TT&R or EXO Travel, unless authorized in writing, may vary, add, or waive any term or condition in this Booking Agreement.

AFFILIATION

Transformational Tours Retreats, 3751 SW 146 Ave., Miramar, FL 33027 (888) 600-7541 is registered with the State of Florida as a Seller of Travel, Registration No. TI64162, is a dba of Halle Eavelyn Enterprises, LLC, and a member of the Gifted Travel Network. Gifted Travel Network, Inc. is registered with the State of Florida as a Seller of Travel, Registration No. ST39093, and with the California Seller of Travel Program, Registration No. 2113317-40. This Agreement is governed by the laws of the State of Florida and Guest shall submit to the courts Miami-Dade County, Florida for the purpose of any suit, action or proceeding.

NOTICE

Written notice of any claim against TT&R must be received no later than fourteen (14) days after the conclusion of the Tour. TT&R address: 3751 SW 146 Ave., Miramar, FL 33027.

Although every effort will be made to deliver the Tour exactly as listed in the Booking Agreement, all terms, and conditions, including fares, schedules, locations, hotels, cruises, and rates are subject to change without notice. Every effort will be made to ensure the price stays the same, but should international currency fluctuate significantly, it may become necessary to pass on that cost to You, the Guest. If this happens, You will receive as much advance notice as possible.

CONTRACT ACKNOWLEDGMENT & AGREEMENT

By signing this Booking Agreement, Guest agrees they have read, understood, and accept to abide by all the Terms and Conditions outlined above. I acknowledge being fully informed about the details of this Tour and of the cancellation policy.
